Description
We are seeking a Home Health Case Manager to oversee and coordinate all aspects of patient care in the home health setting. The Case Manager will be responsible for developing individualized care plans, conducting in-home assessments, collaborating with interdisciplinary team members, and ensuring compliance with regulatory standards.
Key Responsibilities
  • Manage a caseload of home health patients.
  • Conduct initial and ongoing assessments to determine the patient's needs.
  • Develop, implement, and update individualized care plans.
  • Monitor patient progress and revise care plans as needed.
  • Educate patients and families on disease process, medications, and care.
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
  • Maintain accurate and timely clinical documentation in accordance with agency policy and Medicare regulations.
  • Ensure continuity of care and advocate for the patient's needs.

Requirements
Qualifications
  • Active RN license in the state of Missouri.
  • Minimum of 1 year of clinical experience; home health experience strongly preferred.
  • Knowledge of OASIS documentation and Medicare guidelines.
