About the Role

To The Rescue is seeking a Registered Nurse (RN) โ€“ Home Health Case Manager to provide skilled nursing care and care coordination for adult patients in their homes. This role offers meaningful one-on-one patient care, professional autonomy, and a healthier work-life balance compared to hospital or facility-based nursing.

Youโ€™ll manage patient care from admission through discharge, working collaboratively with physicians, therapists, patients, and families to help individuals remain safe, independent, and comfortable at home. We offer a supportive team environment, realistic expectations, and a $15,000 sign-on bonus for full-time RNs .

What Youโ€™ll Do

Provide skilled nursing visits in accordance with physician orders and individualized plans of care

Complete comprehensive patient assessments and ongoing evaluations

Develop, implement, and update care plans in collaboration with the care team

Coordinate services with physicians, therapists, patients, and caregivers

Educate patients and families to support safety, independence, and health outcomes

Document care accurately and timely according to agency and regulatory standards

Communicate changes in patient condition to the appropriate team members

What Weโ€™re Looking For

Active Registered Nurse (RN) license in the state of Iowa

Graduate of an accredited school of nursing (ADN or BSN) At least 1 year of nursing experience (home health experience preferred, not required)

Strong assessment, communication, and time management skills

Ability to work independently while collaborating with a multidisciplinary team

Valid driverโ€™s license and reliable vehicle
