Home Care Scheduler – San Jose, CA

Location: San Jose, CA
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours: 40 hours per week
Position Type: Full-Time Contract
Work Setting: 100% On-Site

Help Keep Senior Home Care Running Smoothly!

We are seeking an experienced Home Care Scheduler to join a compassionate and fast-paced senior care team in San Jose, CA.

This role is perfect for someone who enjoys coordinating caregiver schedules, solving last-minute staffing challenges, and communicating with patients, families, and field staff. You will play an important role in ensuring vulnerable seniors receive reliable, timely care in their homes.

What You'll Do
  • Coordinate daily schedules and assignments for Home Care Assistants and field staff

  • Adjust schedules due to call-outs, patient needs, cancellations, and other changes

  • Communicate scheduling updates with patients, families, caregivers, and team members

  • Verify required personal-care authorizations within the EMR

  • Track completed, cancelled, missed, and rescheduled home visits

  • Collect and monitor Care Logs and timesheets

  • Make reminder calls and provide scheduling instructions as needed

  • Maintain accurate scheduling and patient records

  • Complete documentation accurately and on time

  • Work closely with Home Care Supervisors, Coordinators, and field staff

  • Maintain patient privacy and confidentiality

  • Assist with changing priorities in a busy home-care environment

What We're Looking For
  • High School Diploma or GED

  • At least 1 year of scheduling experience within a home health or home care agency – REQUIRED

  • Experience coordinating caregivers or other home-care field staff

  • Excellent communication and customer-service skills

  • Strong organization and multitasking abilities

  • Ability to handle frequent scheduling changes and competing priorities

  • Comfortable working independently and as part of a team

  • Compassionate and professional when interacting with seniors and their families

Preferred Qualifications
  • 1+ year of EMR experience

  • Experience with healthcare scheduling software

  • Previous experience as a Home Care Scheduler, Home Health Scheduler, Staffing Coordinator, Scheduling Coordinator, Care Coordinator, or Home Care Coordinator
