Job description

Job Summary: Is responsible in handling case management of home health patients and the assigned staff team to ensure effective and quality delivery of home health care services in accordance with the State and Federal Regulations and the Agency's Policies and Procedures. He/she coordinated plan of care, patient care and physician's orders with assigned team members of RNs, LVNs, Medical Social Workers, Certified Home Health Aides and Rehab Services.

Essential Functions:

Acts as an effective liaison to the home health team to ensure continuity and congruity of home health service in accordance with the patient's plan of care,

Oversees a team of RNs, LVNs, and CHHAs to ensure effective and quality home health care services,

Provides professional home health care case management under the direction of the Director of Patient Care Services or Assistant Director of Patient Care. Communicates appropriate status/condition changes following Agency's Policies and Procedures,

Works collaboratively with others to achieve goals, solve problems, and meet established organizational objectives and management of patients,

Responsible for the overall management of assigned patients including the provision of direct care, effective visit utilization management and control,

Coordinates with physicians, other health care professionals - rehab therapists, social services in developing and coordinating plan of care,

Coordinates all information on patient referrals, plan of care, patient care and physician orders,

Assists in establishing the initial Plan of Care and provides supplies and equipment arrangements for the client,

Ensures implementation of a correct, timely and complete clinical documentation in accordance with the Agency's Policies and Procedures and State and Federal regulatory requirements,

Communicates with clinicians and provides training and guidance following compliance with the Agency Policies and Procedures,

Participates in home health case conferences and other Agency meetings,

Provides guidance to Agency home health staff, other health care professionals, patients and families related to appropriate and available health care resources,

Adheres and implements compliance to HIPAA for protection of confidentiality of all written and computer files,

Promotes and participates in the development and execution of the Quality Assurance Performance Improvement (QAPI) Program and activities,

Other related duties and responsibilities that may become necessary or appropriate to meet the clinical care and compliance needs of teh Agency.

Knowledge, Skill, and Experience:

RN with current license to practice in the state of California,

Minimum experience required: 1 year of home health case management experience,

Knowledge of home health regulatory and reimbursement requirements preferred,

Strong coordination, multi-tasking, telephone skills, organizational and interpersonal skills with sensitivity toward Medicare population required,

Computer skills required both clinical and clerical programs, with computer and Microsoft Office skills, must be able to type a minimum of 45 words per minute, and have a working knowledge on medical terminology,

Excellent verbal and written communication skills,

Ability to work independently on assigned tasks as well as accept direction on given assignments,

Ability to supervise and direct members of home health care team,

With great organizational and time-management skills.
