Part-Time / PRN Home Health Aide (HHA/CNA)

Location: Lake County amp; Porter County, Indiana

We are seeking compassionate, dependable, and patient-focused Home Health Aides (HHA) and Certified Nursing Assistants (CNA) to join our growing team on a Part-Time/PRN basis. This role provides one-on-one care in patients' homes, helping individuals maintain their independence, dignity, and quality of life.

Job Responsibilities

As a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), you will:

  • Provide personal care services in patients' homes according to the established plan of care
  • Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility
  • Support patient safety, comfort, and independence
  • Observe, document, and report changes in patient condition to the supervising nurse or clinical team
  • Assist with transfers, ambulation, and mobility support
  • Provide medication reminders and other health-related tasks as permitted by state regulations and agency policy
  • Maintain a clean, safe, and healthy home environment
  • Educate family members and caregivers on appropriate care techniques when applicable
  • Follow infection control, safety, and confidentiality standards
  • Comply with all agency policies, procedures, and Medicare home health guidelines
QualificationsRequired
  • Current Certified Nursing Assistant (CNA) certification or Home Health Aide (HHA) certification (as applicable)
  • Valid driver's license, reliable transportation, and auto insurance
  • Ability to travel throughout Lake County and Porter County, Indiana
  • Strong verbal and written communication skills
  • Ability to work independently in a home care environment
  • Ability to perform physical caregiving duties, including lifting, transferring, and assisting patients with mobility
Preferred
  • Previous experience in home health, hospice, private duty, assisted living, long-term care, or skilled nursing
  • Experience providing personal care and ADL assistance
  • Knowledge of patient safety and infection prevention practices
