Required: Bachelor's or Master's degree in social work and two years experience in social work in a mental health or medical setting.

AuthoraCare Collective is currently seeking a Social Worker Home Care supporting our Greensboro service area. This is a full-time position; Monday - Friday 8:00am - 5:00pm.

Job Summary:

Responsible for providing intervention and assistance to the Social Work Department which is consistent with basic generalist social work practice. This includes: resource assessment and planning, intervention, evaluation, case management, information and referral, supportive presence/supportive counseling, case management, advocacy, community resource and client. Also included is long-term care planning to consist of LTC placement, completion of appropriate documentation i.e. FL-2s and obtaining PASRRs.Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
