This role offers the unique opportunity to work on-site, directly within the homes of your patients.   • Utilize evidence-based methods to treat diverse patient populations, including orthopedic, pediatric, neurologic, and sports medicine cases, enhancing their functional capabilities.
  • Deliver therapeutic interventions at the patientโ€™s home, using specialized procedures and techniques to facilitate rehabilitation.
  • Foster effective communication with patients and their caregivers, providing education and guidance to ensure understanding and adherence to treatment protocols.
  • Maintain patient records with detailed documentation of progress and patient interactions in compliance with regulatory standards.
  • Collaborate with the broader healthcare team to share insights and improve comprehensive patient care, ensuring alignment with best practices.
  • Achieve utilization goals and contribute to the overall operational objectives of Advent Care.
Patient Population / Client Focus

In this role, you will work with a diverse range of clients, primarily focusing on home healthcare. Typical patients may include those recovering from surgeries, individuals with chronic conditions, children with developmental needs, and athletes recovering from sports injuries. Your ability to connect with each patient personally will be crucial in helping them to achieve their personal rehabilitation goals.

Qualifications
  • Graduate of an APTA or CAPTE-accredited physical therapy program.
  • Bachelor's degree in Physical Therapy is required; a Doctorate of Physical Therapy (DPT) is preferred.
  • Valid state license to practice physical therapy and current CPR certification.
  • Proven written and verbal communication skills, with the ability to effectively educate patients and caregivers.
  • Strong organizational skills and competence in computer systems for documentation and scheduling.
  • Ability to establish and nurture relationships with patients, enabling them to exceed their functional goals.
Preferred Experience
  • A minimum of 2 years of professional physical therapy experience in a clinical or home health setting is preferred.
  • Experience in working with a multidisciplinary team to optimize patient outcomes.
Compensation and Benefits
