As a Physical Therapist at Elite Flex, you will work directly in patients' residences, allowing for personalized care that engages family members and caregivers. Our flexible scheduling provides you with a work-life balance, where you will typically operate Monday to Friday from 2 PM to 8 PM. Your primary duties will include:

• Delivering patient-centered care by managing the therapeutic process for assigned patients in collaboration with physicians and the broader care team.
• Conducting thorough patient assessments and participating in designing personalized care plans that focus on goal achievement and patient engagement.
• Implementing therapeutic interventions and enhancing patient mobility and function as per the physician's plan of care.
• Providing skilled therapeutic care within the patient's home, employing evidence-based techniques and procedures to effectuate tangible improvements in condition and function.
• Instructing patients and their families on the appropriate use of therapeutic modalities, exercises, and assistive devices to ensure continuity of care post-therapy sessions.
• Prioritizing your caseload effectively while coordinating your daily staffing needs, ensuring seamless patient assignments and transition of care.

Your caseload will include diverse adult and pediatric patients who may be recovering from surgeries, managing chronic conditions, or in need of rehabilitation following injury. • Must possess a valid license to practice as a Physical Therapist in the state of California.
• A Doctor of Physical Therapy (DPT) degree is required.
• Basic Cardiac Life Support (BCLS) certification is required within the first 30 days of employment.
• No previous experience is necessary, making this an excellent opportunity for recent graduates to start their careers in a supportive environment.

• Previous internships or clinical rotations in home care or outpatient settings are desirable.
• Experience with pediatric and geriatric populations is a plus but not mandatory.
