Position Overview

We are seeking a dedicated and skilled Home-Based Physical Therapist to join our dynamic team at Heal & Revive Chiro Clinic in Baltimore, Maryland. This full-time position offers a unique opportunity to work within a home care environment, where you will bring physical therapy directly to the clients in their residences. Work Environment

This role requires an on-site presence within the homes of our clients across Baltimore and its surrounding areas. You will work primarily Monday through Friday, focusing on direct interactions with patients and collaborating with their families to ensure optimal care delivery. Key Responsibilities

As a Home-Based Physical Therapist, your responsibilities will include:

  • Designing and directing individualized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring functional capabilities, in alignment with physician-prescribed home care.
  • Conducting thorough evaluations of clients' physical abilities, utilizing diagnostic and prognostic tests to assess muscle, nerve, joint, and functional capabilities.
  • Overseeing physical therapy assistants in implementing therapy plans, assuring adherence to established care protocols.
  • Evaluating and adapting the home environment to facilitate rehabilitation, providing actionable recommendations to increase client independence.
  • Documenting patient evaluations, care plans, and progress notes to maintain comprehensive clinical records.
  • Participating actively as a member of the interdisciplinary team, contributing to case conferences and collaborating with various healthcare professionals to ensure holistic patient treatment.
  • Instructing clients and their families on physical therapy techniques, the use of therapeutic equipment, and strategies to support ongoing physical wellness.
  • Monitoring and reporting any changes in client conditions, suggesting modifications to treatment plans as needed.
Patient Population / Client Focus

You will primarily work with a diverse patient population that includes elderly individuals requiring rehabilitation after surgeries or injuries, adults recovering from physical impairments, and pediatric patients with developmental concerns. Your keen understanding of the unique challenges that come with delivering care in a home setting is crucial in fostering trust and achieving positive therapeutic outcomes.

Qualifications

To be successful in this role, candidates should possess:

  • A Bachelor's Degree in Physical Therapy from an accredited program.
  • Current physical therapy licensure in good standing in the state of Maryland.
  • A valid driver's license and valid automobile insurance.
  • Up-to-date CPR certification.
Preferred Experience

We prefer candidates who have:

  • At least one year of experience practicing as a physical therapist, with a strong emphasis on home care or acute care settings.
  • Proven ability to work collaboratively as part of a healthcare team, demonstrating effective interpersonal and communication skills.
