About the Role:

The Home Health Aide plays a critical role in providing compassionate and personalized care to individuals who require assistance with daily living activities in their own homes. This position focuses on enhancing the quality of life for patients by supporting their physical, emotional, and social needs while promoting independence and dignity. The aide will work closely with healthcare professionals and family members to implement care plans tailored to each patient's unique requirements. Through attentive monitoring and timely reporting, the aide ensures that any changes in the patient's condition are promptly addressed. Ultimately, this role contributes significantly to the well-being and comfort of patients receiving long-term or short-term home care services.

Minimum Qualifications:

  • High school diploma or equivalent.
  • Completion of a state-approved Home Health Aide training program.
  • Certification as a Home Health Aide (HHA) or equivalent credential as required by the state.
  • Basic knowledge of patient care techniques and safety protocols.
  • Ability to pass background checks and meet health screening requirements.

Preferred Qualifications:

  • Previous experience in assisted living, long-term care, or home care settings.
  • CPR and First Aid certification.
  • Familiarity with electronic health record (EHR) systems and computer data entry.
  • Strong communication skills to effectively interact with patients, families, and healthcare teams.
  • Additional training or certification in specialized care areas such as dementia or palliative care.

Responsibilities:

  • Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support.
  • Monitor patientsโ€™ health by taking vital signs and observing physical and emotional conditions, reporting any changes to healthcare supervisors.
  • Provide medication reminders and assist with prescribed therapeutic exercises as directed by healthcare professionals.
  • Maintain a clean, safe, and sanitary environment in the patientโ€™s home to prevent infections and promote comfort.
  • Document patient care activities accurately using computer data entry systems and maintain confidentiality of all patient information.
  • Work independently with minimal supervision while managing time effectively to prioritize patient needs and care tasks.
  • Support patients with meal preparation, feeding assistance, and ensuring proper nutrition and hydration.
  • Offer companionship and emotional support to patients, fostering a positive and respectful relationship.
