Work Environment

This position is based onsite as part of our home care program, which allows you to bring therapy services directly into the homes of those we serve. This setting not only ensures a personalized approach to patient care but also helps foster strong relationships with patients and their families in a comfortable and familiar environment.

Key Responsibilities
  • Conduct thorough evaluations of patients in their homes, developing personalized treatment plans tailored to their specific needs.
  • Utilize a range of therapeutic techniques including exercise, manual therapy, and specialized equipment to manage pain and enhance mobility.
  • Monitor and document patient progress meticulously, adjusting treatment plans as necessary to meet therapeutic goals effectively.
  • Collaborate with caregivers, families, and interdisciplinary teams to educate them on treatment modalities and assistive devices necessary for ongoing support.
  • Stay current in the field by engaging in continuing education opportunities, attending workshops, and participating in professional societies.

Patient Population / Client Focus

Qualifications
  • A Doctorate in Physical Therapy (DPT) is required.
