This position is based onsite as part of our home care program, which allows you to bring therapy services directly into the homes of those we serve. This setting not only ensures a personalized approach to patient care but also helps foster strong relationships with patients and their families in a comfortable and familiar environment.

• Conduct thorough evaluations of patients in their homes, developing personalized treatment plans tailored to their specific needs.
• Utilize a range of therapeutic techniques including exercise, manual therapy, and specialized equipment to manage pain and enhance mobility.
• Monitor and document patient progress meticulously, adjusting treatment plans as necessary to meet therapeutic goals effectively.
• Collaborate with caregivers, families, and interdisciplinary teams to educate them on treatment modalities and assistive devices necessary for ongoing support.
• Stay current in the field by engaging in continuing education opportunities, attending workshops, and participating in professional societies.
• Foster a positive and encouraging environment that helps patients feel supported and confident in their recovery journey.

Our client base includes a diverse range of individuals, from young children to older adults, each facing different challenges related to recovery from illness, injury, or surgery. As a Physical Therapist, you will work closely with these patients, developing relationships that are key to their physical rehabilitation journey. You will be instrumental in helping them achieve their health and wellness goals in the familiar surroundings of their own homes.

• A Doctorate in Physical Therapy (DPT) is required.
• A valid licensure to practice as a Physical Therapist in the State of Florida.
• Strong verbal and written communication abilities to effectively interact with patients and their families.
• Proficiency in computer skills for documentation and treatment tracking.
• Ability to work independently while remaining highly flexible and adaptable to the dynamics of a home care environment.

Although we welcome new graduates, we prefer candidates who have at least two years of experience in a clinical setting. Previous experience in home health or outpatient therapy is a plus, as it prepares you to address the unique challenges presented in the home care setting.
