Continuum Home Health LLC is a nurse-led home health agency providing compassionate, patient-focused care throughout the Houston region. We deliver skilled clinical services in the comfort of patientsโ€™ homes, helping individuals maintain independence, restore mobility, and improve overall quality of life.

Duties

  • Conduct comprehensive assessments of patientsโ€™ physical conditions, including mobility, strength, and functional abilities.
  • Develop and implement individualized care plans tailored to each patientโ€™s needs, goals, and medical history.
  • Deliver targeted physiotherapy interventions such as trigger point therapy, orthopedics-focused treatments, and geriatrics care to optimize recovery outcomes.
  • Manage discharge planning processes by coordinating with healthcare teams to ensure smooth transitions from inpatient or outpatient settings to home care.
  • Educate patients and their families on exercises, injury prevention strategies, and physiologic concepts to promote self-management.
  • Document all patient interactions accurately using Electronic Medical Record (EMR) systems, maintaining detailed records of progress and treatment adjustments.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care across acute care, hospital, outpatient clinics, and community settings.

Experience

  • Experience in home health or community-based care preferred.
  • Background in inpatient or hospital settings beneficial for acute care decision-making.
  • Outpatient/clinic experience in orthopedics, geriatrics, and general rehabilitation strongly valued.
