Job description

H2 Homecare is an Atlanta-based pediatric homecare company looking for a dedicated and compassionate RN Case Manager to join our clinical team. This role is responsible for coordinating patient care, ensuring quality services, and serving as a liaison between patients, families, physicians, and caregivers. The RN Case Manager plays a vital role in helping patients receive exceptional care while maintaining compliance with clinical and regulatory standards.

The ideal candidate is an organized, compassionate Registered Nurse with home care experience, excellent communication skills, and a passion for advocating for medically complex pediatric patients. This position is essential to ensuring continuity of care while supporting families and our clinical team.

Responsibilities

  • Conduct monthly in-home patient visits to assess patient needs and ensure quality of care
  • Coordinate patient care plans in collaboration with physicians, caregivers, therapists, and families
  • Complete patient admissions, recertifications, care updates, and required clinical documentation
  • Ensure compliance with company policies, state regulations, and clinical standards
  • Serve as the primary clinical contact for assigned patients and families
  • Collaborate with office staff to coordinate staffing and continuity of patient care
  • Provide clinical guidance and support to field staff as needed
  • Participate in care conferences and communicate updates with the interdisciplinary team

Qualifications

  • Current and active Registered Nurse (RN) license
  • Minimum of 2 years of home care experience required
  • Preferred: 2 years of pediatric home care experience
  • Preferred: 2 years of GAPP experience
  • Strong assessment, communication, organizational, and problem-solving skills
  • Ability to work independently and effectively manage multiple patients and priorities
  • Compassionate, patient-focused approach with excellent customer service skills
