Overview

As a home health Physical Therapist, you would be responsible for conducting one-on-one patient care and patient/family teaching in the home setting. You will work directly with patients in their homes, assessing their needs, developing personalized treatment plans, and implementing therapeutic interventions to optimize their functional abilities.

Requirements
  • Graduate of an accredited physical therapy educational program
  • Current Physical Therapist (PT) state license within the state of practice
  • Current CPR certification
  • Excellent organizational, time-management, written and verbal communication skills
  • Reliable transportation, valid driver's license and current automobile insurance
