REQUIREMENTS
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required
- CHPN certification - Preferred
- Proof of current CPR certification - Required
- Current driver's license - Required
- Reliable transportation and insurance - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2+ years Home health experience - Required
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
- Bachelor's of Science in Nursing - Preferred
ROLES & RESPONSIBILITIES
- Assess home health and/or hospice patients in various situations to identify their physical, psychosocial, and environmental needs.
- Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families on care, diet, medications, and home safety.
- Monitor and assess pain management needs utilizing appropriate techniques.
- Supervise and provide clinical direction to assistive personnel to ensure service quality.
- Provide effective communication and serve as a liaison between patients, families, team members, and other healthcare professionals.
