Program Evaluator, Independent Contractor

Who We Are:

Constellation Quality Health is a non-profit health care quality consultancy and QIO-like Entity certified by Centers for Medicare and Medicaid Services (CMS) founded by physicians in 1983. Headquartered in North Carolinas Research Triangle, we offer an array of quality improvement, clinical review, audit, technical, and consulting services and solutions to improve care delivery, system performance, and patient outcomes.

What Youll Do:

Constellation Quality Health seeks an experienced Program Evaluator, Independent Contractor to provide independent evaluation leadership and technical expertise for a Cardiovascular Health Program grant. This engagement supports implementation across four program lines and focuses on strengthening evaluation design, data reporting, and partner support in alignment with CDC cooperative agreement requirements.

The selected candidate will operate as an independent contractor and will be responsible for delivering agreed-upon evaluation products and services within established timelines.

We expect you to:

  • Lead implementation of the CDC Framework for Program Evaluation in Public Health and ensure evaluation activities align with cooperative agreement requirements, performance measures, and reporting timelines.
  • Develop and maintain evaluation plans, logic models, performance measures, and data collection strategies across four program lines.
  • Prepare and submit required CDC deliverables, including Annual Performance Reports, evaluation reports, continuation application content, and required data submissions.
  • Provide evaluation-related technical assistance to partners and stakeholders, including development of tools, templates, reporting processes, and capacity-building resources.
  • Analyze qualitative and quantitative data to assess program reach, implementation, and outcomes; translate findings into actionable insights and visual reports.
  • Develop presentations, impact summaries, and dissemination materials; represent program evaluation activities in meetings, learning sessions, and regional or national conferences as requested.
  • Collaborate with program leadership and partners to align evaluation efforts, strengthen shared metrics, and support data-informed decision-making.
Engagement Structure:
  • This is a remote, project-based independent contractor engagement.
  • The anticipated level of effort is approximately 2532 hours per week.
  • Up to 25% travel within the Southeastern United States may be required for site visits, partner engagement, and conferences, including occasional overnight travel.
  • Contractors are responsible for managing their own schedule to meet agreed-upon deliverables and deadlines.
  • Candidates located in the Southeastern region are preferred.

Our requirements for this role:

  • Advanced degree (masters or higher) in public health, epidemiology, evaluation, biostatistics, or related field; OR bachelors degree with extensive relevant experience
  • Minimum 5 years of experience in public health or healthcare program evaluation
  • Demonstrated expertise using the CDC Framework for Program Evaluation in Public Health
  • Experience supporting federally funded or CDC-funded programs strongly preferred
  • Experience developing evaluation plans, logic models, and performance measures
  • Experience providing technical assistance to community-based and clinical partners
  • Strong data analysis and reporting skills with proficiency in Microsoft Office and data visualization tools
  • Experience developing and delivering presentations to diverse audiences
  • Experience preparing conference abstracts, posters, or presentations preferred
  • Demonstrated ability to manage multiple deliverables independently

Compensation & Contract Terms

  • Hourly rate: $40 - $45 per hour, commensurate with experience
  • This engagement is funded through a federally supported grant. Continuation is contingent upon ongoing grant funding, program priorities, satisfactory performance and mutual agreement.
  • Funding levels, scope of work, and duration may be modified based on federal guidance, program needs, and availability of funds.
  • The selected consultant will operate as an independent contractor and will not be considered an employee. The contractor will be responsible for their own taxes, benefits, and business expenses, unless otherwise specified in a written agreement.
  • The contractor will be responsible for their own business insurance which may include general liability, professional liability, cyber insurance, automotive insurance and other forms of insurance, unless otherwise specified in a written agreement.
