Job description

Summary: The LCFS Home Based Life Skills Clinicians are responsible for monitoring and providing mental health education/training services to youth with severe emotional disturbances and their families referred to LCFS through the Indiana Department of Child Services and may work with adult clients as part of the adult treatment team.

The LCFS Home Based team provides services concentrating on the East side of Marion County, Hamilton County, Hancock County & Shelby County.

Primary/Essential Functions:

  • Client/Group Services: Participates in development of curricula for individual and group services that meet the individual needs of the client/family served. Trains and assists clients and/or family in the developing of living skills, individually or in groups. Monitors individual client/family outcomes as related to skill acquisition and training. Provides services in homes and/or community sites. When necessary, will transport clients in personal vehicle.
  • Assessments and Evaluations: May assist with gathering data for comprehensive bio-psychosocial assessments. Evaluate functioning and identify ongoing risk factors and communicates these to the client/family team.
  • Care Plan: Communicates need to update plan of care as it is related to skill acquisition and training. Participates in the development of individualized treatment plans in collaboration with the client/family team. Collaborates as an active multidisciplinary team member. Completes documentation in EMR in timely manner per LCFS policy.
  • Department Accountabilities: Completes other documentation (e.g.time cards, PTO requests, mileage reports) according to LCFS policy and procedure. Participates in Aggressive Behavior Management appropriate to the population served. See Behavioral Care Services policy: Provision of Aggressive Behavior Management Training of Staff. Recognizes and commits to critical importance of customer satisfaction, both internal and external customers. Actively involved in professional development and education. Skills & Experience:

  • 4 year / Bachelor's Degree (preferred)
  • In lieu of the above education requirements, a combination or experience and education may be considered
  • 1+ years of experience in a human services related field involving skills training and case management and/or education field (preferred)
  • Ability to independently manage time and priorities
  • Ability to independently resolve problems
  • Ability to travel between locations
  • Ability to use patient care skills to provide an exceptional experience
  • Must have interpersonal skills including motivation, flexibility, persistence, and patience
  • Must have the ability to operate a motor vehicle in accordance with LCFS policy (Current Indiana Driver's License & Full Coverage Insurance)
  • Must have the ability to relate in a non-judgmental and culturally competent manner
  • Must have the knowledge of behavioral health competencies specific to population being served
  • Must possess effective organizational skills
  • Must possess knowledge and creativity in regard to community resources
  • Must possess knowledge of or readiness to learn strengths oriented Recovery and Resiliency Models
  • Must value client/family centered care and able to apply strengths based needs assessment
  • Ability to communicate effectively both orally and in writing

  • Able to work hours needed to accomplish assigned tasks, to include evenings, weekends, & holidays when necessary
  • Use of personal vehicle (mileage reimbursement) to travel to client homes & work with clients inside their residence.
