Job description

Job Summary
Home Care Now is seeking a compassionate and clinically strong Home Health RN to provide one-on-one care in someones home. This role is ideal for a nurse who values autonomy, patient relationships, and the ability to deliver skilled care in a home-based setting rather than a facility environment.
What You'll Do
  • Assess patient condition, vital signs, and response to treatment
  • Administer medications and perform wound care as ordered
  • Educate patients and families on care plans and home health best practices
  • Coordinate with physicians, family members, and the broader care team
  • Complete accurate, timely documentation for each visit

What We're Looking For
  • Active RN license in the state of employment
  • Current CPR or BLS certification
  • Level 2 Background
  • Vent, teach gtube experience
  • Strong assessment and communication skills
  • Ability to work independently in a home-based care setting
  • 1 year Home health experience preferred
