Overview of the Home-Based Primary Care Physician Role:

The Home-Based Primary Care (HBPC) Physician and Patient Care Manager (PCM) will operate under general supervision to deliver primary care services to veteran beneficiaries. Responsibilities encompass the examination, diagnosis, prevention, and treatment of various medical conditions, along with the oversight of non-credentialed healthcare providers. Candidates must possess Board Certification or Board Eligibility in Family Practice and hold a BLS certification.

Key Responsibilities:
  • Act as the Home-Based Primary Care (HBPC) Physician and Primary Care Manager (PCM) for veterans.
  • Deliver primary care services, including examination, diagnosis, prevention, and treatment across multiple medical fields (medical, surgical, gynecological, pediatric, orthopedic).
Clinical Duties:
  • Perform physical examinations to assess the presence and severity of diseases, injuries, or conditions.
  • Order and interpret diagnostic tests (such as laboratory tests and x-rays).
  • Diagnose conditions and create treatment plans, including referrals to specialists or hospitals.
  • Manage complex cases by consulting with higher authorities for diagnostic or consultative advice.
  • Supervise non-credentialed healthcare providers and potentially oversee Nurse Practitioners (NPs) or Physician Assistants (PAs).
Procedures:
  • Reduce simple fractures, conduct emergency deliveries, and perform joint aspirations/injections.
  • Carry out ECGs and interpret initial results, as well as basic radiographs.
  • Administer regional anesthesia, splinting/casting/immobilization of simple fractures, crisis intervention, and provide basic life support (BLS).
