Position: Clinical Therapist - 50% Home-Based / 50% In-Office & Telehealth (LMSW or LLMSW)
Salary: Competitive salary based on experience
Education/Licensure: Master's degree in Social Work and active Michigan LMSW or LLMSW licensure required.

Overview of the Role

Jewish Family Service of Metro Detroit (JFS) is seeking a compassionate and skilled Clinical Therapist to provide assessment, individual, family, and group therapy services across home-based, in-office, and telehealth settings. This role is designed as approximately 50% home-based clinical care and 50% in-office and telehealth services. The therapist will support clients through a range of mental health needs while increasing access to high-quality, trauma-informed care in the settings that best meet client needs.

Role & Responsibilities
  • Provide evidence-based individual, family, and group therapy in clients' homes, JFS office settings, and via telehealth.
  • Maintain a clinical schedule that supports the expected balance of approximately 50% home-based services and 50% in-office/telehealth services.
  • Travel throughout the assigned service area to provide home-based services; mileage is reimbursed for approved work-related travel.
  • Complete biopsychosocial assessments, risk assessments, treatment plans, safety plans, and ongoing reassessments.
  • Provide crisis intervention, safety planning, care coordination, and referrals to additional supports as appropriate.
  • Collaborate with clients, families, onsite psychiatrists, physicians, schools, case managers, and community partners to support whole-person care.
  • Connect individuals and families to appropriate community resources, serving as a liaison and advocate when needed.
  • Maintain timely and compliant documentation in the electronic health record, including progress notes, treatment plans, records, and reports.
  • Participate in agency in-service programs, workshops, supervision, team meetings, and quality improvement activities.
  • Offer flexible scheduling, including up to two evening sessions per week for clients unable to attend daytime appointments.
  • Protect client confidentiality and maintain trust by securing sensitive information in accordance with agency protocols
Required Qualifications
  • Master's degree in Social Work and active LMSW or LLMSW license in the state of Michigan.
  • Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area.
  • Strong clinical judgment, documentation skills, communication skills, and ability to work independently in community-based settings.
  • Ability to provide services in home-based, office-based, and telehealth formats.
Preferred Qualifications
  • Experience providing therapy to children, adolescents, adults, families, and/or older adults.
  • Experience working with older adults, caregivers, or individuals navigating aging-related transitions preferred.
  • Experience with trauma-informed care and evidence-based practices such as CBT, DBT, EMDR, ACT, or Motivational Interviewing.
  • Experience providing home-based, community-based, or integrated behavioral health services.
