Holy Family & Marriage Services is seeking a full-time Licensed Clinical Social Worker (LICSW) to join our dedicated team at our Hospice Center in Graham, Washington. As part of our hospice team, you will work remotely, allowing you to maintain a healthy work-life balance. This role requires a self-motivated individual who can perform duties effectively from a home office while engaging with patients and families through telehealth services. • Conduct comprehensive psychosocial assessments to determine the mental health needs and strengths of patients while documenting findings for ongoing support.
• Provide evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), tailored to each patient’s unique circumstances.
• Collaborate with a multidisciplinary team, including physicians and nurses, to develop and adjust personalized care plans that align with patients’ health goals.
• Educate patients and their families on diagnoses, treatment options, and medication management; empower them to make informed decisions about their care.
• Conduct individual, family, and group therapy sessions, utilizing proven techniques to foster healing and coping strategies.
• Advocate for patients navigating the complex mental health system, ensuring access to necessary resources and support.
• Facilitate psychoeducation and therapeutic groups to address specific patient needs, promote healthy coping mechanisms, and foster community support.
• Participate in discharge planning meetings, coordinating with all stakeholders to ensure smooth transitions and follow-up care.

This position will primarily involve supporting hospice patients and their families, who are facing challenging emotional and psychological situations during end-of-life care. The social worker will address a diverse range of issues, including grief, loss, anxiety, and depression, ensuring that each patient receives holistic care that honors their values and respects their choices.

• A Master's degree in Social Work from an accredited institution.
• Current and active state licensure as a Licensed Clinical Social Worker (LICSW).
• Strong foundation in mental health disorders, treatment modalities, and crisis intervention techniques.
• Ability to work effectively in a multidisciplinary team, collaborating with various healthcare professionals to achieve optimal patient outcomes.

• Previous experience in hospice, palliative care, or a related field is preferred.
• Familiarity with telehealth platforms and electronic health record (EHR) systems.
• Experience in providing grief counseling and support to families undergoing bereavement.

Holy Family & Marriage Services offers a competitive annual salary ranging from USD 80,000 to 104,000, commensurate with experience and qualifications. In addition, our comprehensive benefits package includes:
