Registered Nurse - Hospice / Palliative Care
We are seeking a compassionate and dedicated Registered Nurse to join our hospice care team, providing high-quality, patient-centered support to individuals and their families during end-of-life care. This role offers the opportunity to make a meaningful difference by delivering personalized care and guidance in a respectful and empathetic manner.
Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized care plans tailored to physical, emotional, and spiritual needs.
  • Coordinate and manage patient visits, ensuring care aligns with clinical directives and patient goals.
  • Administer medications and follow physician orders accurately and in a timely manner.
  • Provide supervision and guidance to nursing support staff, including LPNs, LVNs, and hospice aides, according to established protocols.
  • Collaborate with an interdisciplinary team to deliver holistic care addressing the full spectrum of patient needs.
  • Educate families and caregivers, offering resources and support to help them navigate the hospice experience.
  • Maintain a compassionate, empathetic, and professional approach in all interactions, upholding high standards of integrity and patient dignity.
Qualifications:
