About the Role:

The Immunization Action Plan Coordinator plays a critical role in enhancing public health by developing, implementing, and managing comprehensive immunization strategies across diverse communities in the United States. This position is responsible for coordinating efforts among healthcare providers, public health agencies, and community organizations to increase vaccination rates and reduce vaccine-preventable diseases. The coordinator will analyze immunization data to identify gaps and barriers, then design targeted interventions to improve vaccine accessibility and acceptance. Collaboration with stakeholders to ensure compliance with federal and state immunization guidelines is essential to the role. Ultimately, the coordinatorโ€™s work contributes to safeguarding population health and supporting national immunization goals through effective program management and community engagement.

Minimum Qualifications:

  • Bachelorโ€™s degree in Public Health, Nursing, Health Administration, or a related field.
  • At least 3 years of experience in immunization program coordination, public health, or healthcare project management.
  • Strong knowledge of immunization schedules, vaccine-preventable diseases, and public health policies in the United States.
  • Proficiency in data analysis and interpretation related to immunization coverage and public health metrics.

Preferred Qualifications:

  • Experience with patient interactions, health records, coordination with providers, and knowledge of EMR systems

Responsibilities:

  • Conduct auditing visits for vaccines for children (VFC, USH, IQIPS).
  • Contact and engage the parents and guardians of under-immunized children.
  • Educate people about the importance of immunization.
  • Identify ways to improve vaccine delivery.
  • Increased immunization rates in the towns or communities they service.
  • Successfully link at-risk children to a โ€œmedical homeโ€ so they receive vaccinations, and their immunization is tracked.
  • Conduct monthly outreach reports for kids.
  • Coordinate with primary and back up vaccine coordinators.
  • Conduct outreach activities as needed.
  • Assist with educational workshops and presentations.
  • Assist with immunization campaigns.
  • Attend trainings and meetings with The Department of Public Health and Hispanic Health Council.
