Home Study/Post Release Services Program LEVEL 3 Regional Case Manager

Reports to: Regional Manager of the Home Study/Post Release Services (HS/PRS) Program

Hours: FT Position – Monday – Friday flexible based on the needs of the HS/PRS Program

This position requires 85% travel.

Holy Family Institute (HFI) is a 125-year-old Catholic organization with an impeccable history of serving children and families in the Pittsburgh Region. We are one of Pittsburgh’s largest social service organizations operating 15 programs nationwide and in three counties in Pittsburgh, Pennsylvania. We are an independent organization guided by Catholic Social Teachings and serving people of all faiths and backgrounds.

General Statement of Duties: Case Managers provide personalized regional services to minors and sponsors. These positions are client home-based and are responsible for assessment, service planning, service coordination, psychoeducation, advocacy, discharge planning, completing required documentation following HFI and the Office of Refugee Resettlement (ORR) policies and guidelines. The RCM is expected to travel and conduct visits in clients’ homes.

Essential Functions:

  • Promotes the Mission and Philosophy of Holy Family Institute.
  • Maintains client confidentiality.
  • Coordinates case management services.
  • Maintains a caseload of up to 35 cases depending on level, program need and/or requests by ORR.
  • Develops assessments, service plans, and coordination of services for clients within required timeframes.
  • Completes required documentation by deadlines and with accuracy.
  • Makes initial contact with clients within timeframes.
  • Completes Home Study Reports.
  • Keeps contact with clients following ORR contact requirements and best practices.
  • Travels to clients’ homes to provide services.
  • Coordinates services with the local organizations and has knowledge of community resources.
  • Develops and maintains professional and supportive relationships with all agency staff, volunteers, ORR, CYF, ICE, VERA, and community.
  • Models and assists in the implementation of the HART and Trauma-Informed Care.
  • Manages unanticipated events and crisis situations in a positive, constructive manner.
  • Utilizes various transportation methods to see clients in their homes within a specific region of the United States.
  • Adheres to ORR, State licensing and HFI policies and procedures.
  • Assists program leadership in the reporting of any additional information as needed for ORR, state licensing, and/or internal measures.
  • Maintains reporting standards of quality and compliance in accordance with HFI standards.
  • Maintains a cooperative attitude with other personnel.
  • Upholds confidentiality regarding client, staff, and agency information in accordance with HIPAA guidelines.
  • Communicates with Regional Manager as to any problems which may arise regarding the operations/functioning of HS/PRS program.
  • Exhibits cultural competency and sensitivity.
  • Participates in required training and development.
  • Adheres to other requests as indicated by the program director.

Minimum Requirements

  • Bachelor’s degree in the behavioral sciences, human services, or social service fields
  • 2-5 years of progressive employment experience in the human service fields. Previous case management experience is highly preferred.
  • Fluency in Spanish (read, write, and speak) is required.
  • Must be at least 21 years of age.
  • Have a valid driver’s license.
  • Have good verbal, written and oral communication skills
