Erie Family Health Centers, a nationally recognized top workplace with 14 sites in Chicago and suburbs, is looking for a valuable addition to our Lending Hands for Life team! The Patient Navigator builds individual and community capacity through outreach, education, counseling, and advocacy. Using referrals and agency data, they identify people who are HIV+, HepC+, or at risk and not in care, and work to reengage them in medical services through calls, texts, letters, home visits, and other outreach methods.

*Compensation is based on each candidate's experience, skills and education within the range identified for the role. Main Duties & Responsibilities
  • Provide accurate information to clients about living with HIV/HepC or taking PrEP, including how to prevent transmission, the benefits and challenges of care and treatment, and available services and how to access them. Deliver health information using culturally appropriate terms and concepts.
  • Accompany patients living with HIV and/or HepC and those patients vulnerable to HIV and HepC to medical or social service appointments as needed.
  • Provide informal counseling, support and follow-up to clients. Help people understand their health condition(s) and develop strategies to improve their health and well-being.
  • Assist with patient navigation, benefits enrollment, and health education.
  • Link vulnerable populations to health care/social service resources.
  • Assist clients with enrollment or re-enrollment in medical care, medical benefits programs and other benefits for which they are eligible.
  • Facilitate communication and client empowerment in interactions with health care/social service systems.
  • Plan and facilitate patient educational groups.

Skills and Knowledge
  • Local travel required.
  • CPR certification required.
  • Bilingual fluency (English/Spanish) strongly preferred.
  • Verbal and written communication skills, including high level of comfort presenting to groups.

Experience
  • A minimum of two years of work experience in health, social or community services preferred.

Education
  • High School diploma or equivalent required
