Multi-State Remote Physician (MD/DO) โ€“ Contracted

Location: Remote โ€” United States (Licensure required in at least 3 of: AL, GA, IL, IN, LA, MS, NC, OH, TN)

THE HIGHLIGHTS

  • Licensure: Candidate must hold valid, unencumbered medical licensure in at least 3 of the following states: AL, GA, IL, IN, LA, MS, NC, OH, TN.
  • Schedule: 1099 flexibility; you set your own hours with at least 10 hours of weekly availability.
    • Current Demand: Mondayโ€“Friday, 9 a.m.โ€“6 p.m. ET
    • Requirement: Provide availability for the next two months by the 15th of each month.
  • Compensation: $40.00 per completed consult + $30.00/hour for non-clinical time; budget for an average of $120/hr (see Compensation & Payment Terms below).
  • Remote Requirements: Must reside in the United States with consistent access to a clean, quiet workspace; solid internet connection; and proficient technical experience.

Perry Health is rethinking chronic care through a fully remote, continuous care model. We provide expert diabetes education and clinical management for Medicare patients, delivered by a comprehensive team of specialized Registered Nurses, Registered Dietitians, and Accountability Coaches.

Our model integrates high-level clinical expertise with real-time data from cellular-connected glucometers and advanced CGM management. By bridging the gap between physician visits with proactive, data-driven support, we deliver life-changing outcomes for our members.


ABOUT THE ROLE

We are seeking an experienced Physician (MD or DO) passionate about remote care delivery. In this role, you will support diabetes education and management by conducting phone visits for initial member consultations. You will be responsible for evaluating diagnoses, medications, and lab results to assess member acuity and validate candidacy for our clinical program. Additionally, you may support the full clinical organization as a point of contact for clinical escalations.


KEY RESPONSIBILITIES
  • Provide call-based care to members in Perry Health's Remote Patient Monitoring (RPM) program.
  • Deliver swift, empathic care to members and assess their clinical competencies.
  • Evaluate medical history and data to determine member eligibility for the clinical program.
  • Serve as a primary clinical point of contact for practice-wide escalations.
  • Collaborate with the broader care team โ€” including RNs, RDs, and Accountability Coaches โ€” to reinforce clinical goals and SMART goal progression.
  • Adhere to Perry Health policies, procedures, and standard operating procedures to ensure ongoing clinical quality and compliance.

QUALIFICATIONS
  • Education: Medical Degree (MD or DO) from an accredited institution.
  • License: Valid, unencumbered state medical license required. Multi-state licensure required โ€” must hold active licensure in at least 3 of the following states: AL, GA, IL, IN, LA, MS, NC, OH, TN.
  • Experience: Minimum 3+ years of experience managing patients with diabetes.
  • Knowledge: Familiarity with continuous glucose monitors (CGMs) and behavior change theory is a plus.
  • Skills: Motivated learner, willing to acquire proficiency with the necessary technology (computers, software applications, phone systems); experience or training in Motivational Interviewing to support sustainable behavior change is a plus.
  • Traits: Creative thinking, an eye for detail, ability to work independently, a positive attitude, and the ability to excel in a fast-paced atmosphere while collaborating with diverse groups at all levels of the organization.
  • Language: Spanish-speaking highly preferred.

COMPENSATION & PAYMENT TERMS

Contractors are paid on a monthly basis following Net 30 payment terms.

  • Clinical Pay: $40.00 per completed consult. Consults are scheduled in 20-minute slots and require a minimum of 15 minutes of engagement time per member โ€” inclusive of live conversation, chart review, and care coordination โ€” for the visit to be billable. Payment is issued for completed, billable visits.
  • Non-Clinical Pay: $30.00/hour flat rate for required meetings and training.

Example: Completing three consults within an hour pays a total of $120 for that hour.
