Job description

We are seeking a Registered Nurse RN to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Responsibilities:

  • Provide skilled nursing services in patients’ homes in accordance with physician orders and state regulations

  • Conduct comprehensive assessments and develop individualized care plans

  • Supervise and monitor patient care to ensure safety, quality, and compliance

  • Coordinate care with physicians, caregivers, and interdisciplinary team members

  • Perform OASIS assessments

  • Leadership and team support 

Qualifications:

  • Previous experience in home health 
  • Ability to build rapport with patients
  • Strong problem solving and critical thinking skills
  • Ensure compliance with Indiana Department of Health Regulations
  • Support survey readiness and participate in mock surveys
  • Maintain accurate clinical documentatiom and chart audits
  • Strong assessment and care planning skills
