We are seeking energetic and detail-oriented Care Coordinators to join our team. In this vital role, you will serve as a key link between clients and providers, ensuring seamless coordination of services and support. Your proactive approach will help improve client outcomes and foster a positive experience for those we serve.

Responsibilities

Care Coordinators provide ongoing assessments and monitoring of the mental health, as well as the physical health of our clients. Extensive onboarding training to prepare you for job duties/tasks including hands-on, shadowing, and staff training is provided.

Qualifications

  • Bachelor's degree in social work, psychology, sociology, or other social/family related science required
  • One (1) years' experience in a behavioral health/human services field preferred

License/Certifications

  • Driver's license required
  • Auto insurance required
