The LPN Healthcare Manager will lead and coordinate nursing services within our community support programs. This vital role involves overseeing patient care, managing clinical staff, ensuring compliance with healthcare regulations, and fostering a compassionate environment for individuals with complex health needs.
Minimum Qualifications:
  • Valid LPN license with current state licensure required; RN licensure considered a plus
  • Extensive experience in critical care environments such as ICU or emergency medicine preferred
  • Proven background in medical office management, long-term care, assisted living, or nursing home settings
  • Hands-on experience with EMR/EHR systems including Epic, Cerner, or Athenahealth; familiarity with ICD-9/10 coding highly desirable
  • Experience administering injections, phlebotomy, ventilator management, and wound suturing preferred
  • Demonstrated ability to lead multidisciplinary teams while maintaining compliance with healthcare regulations such as HIPAA and NCQA standards
Job Responsibilities:

  • Oversee clinical assessments, vital signs monitoring, medication administration, and medical documentation in accordance with HIPAA standards
  • Coordinate medical procedures such as IV infusions, catheterizations, tube feedings, and sterile techniques while ensuring safety protocols are followed
  • Manage EMR (Electronic Medical Records) systems like Epic or eClinicalWorks to maintain accurate patient records and documentation
  • Conduct patient assessments, triage calls, and coordinate urgent or emergent care needs in collaboration with healthcare providers
  • Develop discharge plans and collaborate with case management teams to ensure comprehensive care continuity
  • Provide training and mentorship to staff on topics such as infection control, HIPAA compliance, ICD coding (International Classification of Diseases), and other clinical best practices
