Job description

Case Management Consultant

Provides excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program. Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes. Serves as a consultant to the healthcare team on specific patient items. Coordinates a continuum of care for a defined population of patients from pre-admission through post discharge. Assures that patients and families proceed efficiently through the course of hospitalization. This unit provides evaluation and treatment for patients of all ages diagnosed with acute psychiatric, behavioral or mental health disorders.

Day Shift, 5 x 8s, 0800 - 1630, no set days of the week. 2 years of experience. Must have Epic experience. This role works on 2 adult MHU units. One Medical Behavioral and the other a typical adult Psy. Having behavioral health background is preferred. Will need Case Management skills as well. No Call. BLS and TEAMS or CPI.
