El/La Enfermero(a) Registrado(a) Administrador(a) de Casos planifica, organiza y dirige los servicios de cuidado en el hogar, así como la comunicación con el médico tratante, el paciente y los familiares de los casos asignados. Proporciona atención de calidad mediante el desarrollo, la implementación y la evaluación de planes de cuidado individualizados para cada paciente.

Actúa como enlace entre la Agencia, el paciente y su familia o cuidadores. Cumple con los estándares profesionales de práctica, las políticas y procedimientos de la Agencia, así como con los requisitos de acreditación y las leyes y regulaciones federales, estatales y locales relacionadas con los servicios de salud en el hogar.

Garantiza, mediante la programación y documentación del personal clínico, que la atención se brinde de acuerdo con las órdenes médicas, el plan de cuidado establecido y las regulaciones que rigen los servicios de salud en el hogar.

  • Licencia vigente como Enfermero(a) Registrado(a) en Puerto Rico.
  • Se prefiere Bachillerato en Ciencias de Enfermería (BSN).
  • Mínimo de tres (3) años de experiencia como enfermero(a) registrado(a) licenciado(a), incluyendo experiencia clínica en cuidado en el hogar.
  • Mínimo de dos (2) años de experiencia en una posición de supervisión, preferiblemente.
  • Experiencia en aseguramiento de calidad en hospitales de cuidado agudo y/o servicios de salud en el hogar, preferiblemente.
  • Atención al detalle.
  • Excelentes destrezas de comunicación verbal y escrita.
  • Recopilación, manejo y análisis de datos.
  • Análisis y resolución de problemas.
  • Planificación y organización.
  • Buen juicio profesional.
  • Toma de decisiones.
  • Orientación al servicio al cliente.
  • Trabajo en equipo.
  • Capacidad para educar y orientar a otras personas.
