The Manager of Care Management, under the guidance of the Director of Care Management, provides leadership, coaching and mentoring of the Care Management department professional and administrative staff in order to support and ensure standards of practice for service delivery and organizational performance strategies are consistent with PHC's Mission, Vision and Values. The manager will also provide Care Management services for hospital patients as appropriate to guide care facilitation, coordination, and transitions of care.

Education

  • Graduate from a recognized, accredited school of nursing Required or
  • Bachelor’s Degree in Social Work, Healthcare Administration, or other related field Required

Work Experience

  • 5 years If RN: Need combination of Nursing and Care Management experience in a hospital setting. Experience must include of leadership/Management experience in acute care setting Required

Licenses and Certifications

  • RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure If candidate is a nurse, then a current registered nurse (RN) license in the state of Georgia (or compact state) Upon Hire Required or
  • LMSW - Licensed Medical Social Worker - State Licensure If candidate is a social worker, then a current unrestricted licensed master social worker (LMSW) licensure in the state of Georgia Upon Hire Required
  • Certified Case Manager Preferred
  • CSWCM - Social Work Case Manager Certification Preferred
