  • Evaluate patients' physical and functional abilities to develop individualized care plans.

  • Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments.

  • Provide hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function.

  • Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques.

  • Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care.

  • Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.

  • Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy.

Requirements:

  • Graduate of an accredited School of Physical Therapy.

  • Current State license as a Physical Therapist, or a multi-state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact.

  • Minimum of two years of experience as a Physical Therapist in a clinical care setting, with experience in Home Health, geriatrics, or medical-surgical preferred.

  • Excellent interpersonal and communication skills.

  • Strong nursing skills as defined by generally accepted standards of practice.

  • Reliable transportation with valid and current auto liability insurance.

  • Ability to work flexible hours and travel locally as needed.

  • Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
