Health & Wellness Director | Full-Time

Lead with Compassion | Inspire Your Team | Elevate Resident Care

At Sage Mountain Senior Living, an Agemark Senior Living community, we're looking for a compassionate and experienced Health & Wellness Director to lead our clinical team and ensure exceptional care for every resident. If you're a licensed nurse who enjoys mentoring others, building meaningful relationships, and making a lasting impact, we'd love to meet you.

What You'll Do

As our Health & Wellness Director, you will:

  • Lead and support the wellness team while fostering a positive, resident-centered culture.
  • Oversee resident assessments, care plans, and clinical documentation.
  • Partner with residents, families, physicians, and community leaders to ensure outstanding care.
  • Ensure compliance with state regulations and Agemark policies.
  • Supervise medication management and nursing services.
  • Hire, coach, train, and develop wellness team members.
  • Respond to resident concerns and emergencies with professionalism and compassion.
  • Identify opportunities to improve resident health, safety, and quality of life.
  • Serve as Manager on Duty as needed.
What We're Looking For
  • Current RN or LPN license in California.
  • Minimum one year of experience working with seniors.
  • Two or more years of leadership or management experience.
  • Strong clinical assessment and medication management knowledge.
  • Excellent communication, leadership, and organizational skills.
  • Ability to build relationships with residents, families, and team members.
  • Comfortable using computer systems and electronic documentation.
  • Flexibility to work occasional evenings, weekends, and on-call responsibilities as needed.
