You will assess resident needs, coordinate care, support healthcare transitions, oversee medication processes, and ensure that care plans and documentation remain accurate and current.

You will also serve as a resource and mentor to the resident care team, helping staff deliver proactive, compassionate care while maintaining compliance with applicable assisted living regulations and company standards.
