Job description

Seeking Nurse Practitioners with 1+ years of experience to conduct In-Home Health Risk Assessments. Bonus opportunities included

What's in it for you?

· Flexibility: Operate on your terms. Set your own hours, 7 days a week between 8am-8pm. The role is based on a 1099 Independent Contractor arrangement.

· Compensation: Earn $100-125 for each completed in-home Health Risk Assessment depending on coverage area and hours of availability given. Each assessment takes 45 mins - 1 hour. Mileage reimbursement and a $25 No-show payment.

· Location: We have extensive volume in these counties: Ottawa, Sandusky, Wood, Lucas

About the Role: In this position, you'll be conducting comprehensive Health Risk Assessments for Medicare Advantage patients. Each visit provides an in-depth understanding of a patient's health and well-being that might be missed during regular office visits. From reviewing their medical history to understanding their social environment, your insights will enhance care quality, bridging multiple care gaps in one visit.

Our Ideal Candidate:

  • Board-certified Nurse Practitioner licensed in Ohio

  • At least 1 year of NP experience.

  • Home Health experience
