Position Details
As a member of the Assertive Community Treatment (ACT) Team, the Mental Health Professional plays a vital role in delivering comprehensive services to individuals with severe and persistent mental illness.
  • Assess, plan, and develop individualized treatment plans for individuals with severe and persistent mental illness.
  • Coordinate and provide therapy, medication management, and resource linkage services.
  • Collaborate with a multidisciplinary team including psychiatrists, nurses, substance abuse professionals, employment specialists, housing specialists, and peer specialists.
  • Conduct therapeutic services in an individual's home or community setting.
  • Utilize personal vehicles for client transportation and community-based work.

Our Assertive Community Treatment (ACT) model supports individuals with serious mental illnesses who may not have experienced success or haven't found effective support within traditional outpatient programs. We offer tailored services emphasizing relationship-building and active involvement in enhancing functioning and symptom management.
Services involve weekly home visits, provided either in individuals' residences or within the community, utilizing personal vehicles for client transportation and community-based tasks.
