Job description

Overview

The YNHHS Population Health Analyst will play an instrumental role in developing the analytics capabilities for the Center for Population Health in support of the Yale New Haven Health Clinical Affiliates and the broader enterprise population health strategy. Specifically, the Population Health Analyst executes the population health analytics approaches and initiatives required to measure performance in value-based arrangements, clinical programs, and network performance management. Under the direction of the department leadership, the Population Health Analyst works within a cross functional team to define the analytic needs of a program or initiative and facilitates the execution of those needs. The incumbent has a demonstrated record of data-driven discovery, intellectual curiosity, and a passion for working with data to answer important questions about clinical care and population health.

Responsibilities
  • Data Analytics
    • Creates reports and dashboards that track multidimensional performance against division or organizational goals.
    • Designs, implements, and supports data modeling to provide insights to drive organizational performance improvement initiatives.
    • Establishes consistent approaches and standards for population health measures, their specifications and related monitoring and measurement.
    • Utilizes advanced querying techniques with thorough knowledge and understanding of data warehouse concepts and content.
    • Develops data visualizations to provide clear and insightful views of performance.
    • Performs benchmarking against local and national standards related to population health, clinical quality and financial performance.
    • Presents data in effective, clear formats tailored to needs of various audiences, including senior leadership, managers, staff, and clinicians.
    • Responsible for routine and ad hoc dissemination of key information to stakeholders.
    • Supports the analytic needs of department or organizational business plans, impact evaluations and and new program proposals.
    • Partners with teams in Quality, Finance, Technology and Operations as well as other Population Health functional areas.
  • Performance Improvement and Project Management:
    • Participates in departmental and organizational performance improvement projects as assigned.
    • Perform data validation and quality assurance checks to ensure completeness and accuracy of data prior to publishing data or moving data to production.
    • Conducts needs analysis related to current operations and makes recommendations to promote innovative use of existing analytics tools
    • Perform assessment to identify and address gaps between data requirements and data availability.
    • Contributes to development of project plans and status reports; ensures project deliverables and timelines are on schedule and of high quality.
    • Documents standard operating procedures (SOPs) and other business requirements outlining data standards and best practices.
  • Network Administration
    • Assists with the integrity of provider data and collaborates with internal and external stakeholders to ensure data accuracy.
    • Routinely perform audits and reviews data to ensure the provider information that is used for administering networks is accurate and complete for the populations the department is responsible for managing.
    • Identify opportunities for operational efficiencies in relation to provider management, building upon lessons learned and scaling to future initiatives
  • Other duties as assigned in support of Population Health and the Clinically Integrated Network, which may include but not limited population health analytics, program evaluation, and regulatory readiness

  • knowledge of and interest in advanced query and database skills such as SQL, Tableau and SAS preferred
  • Experience with either claims data or electronic medical record software is preferred
  • Comfortable learning new software tools and other project specific software
  • Strong communication, presentation, relationship building and organization skills, required.

PHYSICAL DEMAND

  • Hybrid position; office in New Haven, CT. Must have own reliable transportation and be able to travel to across the health system delivery network and to our partner organizations. Some early morning or late meetings may be needed to accommodate physician schedules.
