Job Summary and Qualifications

The Case Management Division Director supports the execution of the HCA case management program across the division. The Division Director is a subject matter expert in all aspects of case management, providing direct onsite support and change management. Facility onsite support includes data analysis, facility gap assessments, and action plan development to achieve established goals. Directs facility case management regarding care coordination/case management and care transition processes across the division. The Division Director ensures that the facilities are in compliance with HCA standards, policies, and regulatory requirements. The Case Management Division Director must possess excellent communication skills, proven motivational capabilities. Must have an executive presence and a demonstrated record of accomplishment for delivering results and attaining goals.

Responsibilities

  • Provides leadership to and actively participates with facility staff to identify, implement, or enhance Case Management programs consistent with HCA and facility strategy
  • Disseminates information accurately and timely with multiple audiences; leads regularly scheduled communication sessions with facility CM Directors
  • Identifies process improvement opportunities; develops, implements, monitors, and revises action plans
  • Identifies potential best-demonstrated practices to explore implementation.
  • Identifies opportunities to establish standards and policies.
  • Collaborates with key stakeholders at corporate and division to explore development.
  • Facilitates Case Management education within the division; assists with orientation and ongoing education of Case Management staff, provides education
  • Identifies and proposes opportunities to improve current technology or identify new technology to enhance performance and productivity.
  • Escalates issues to division and corporate leadership.

Education & Experience

  • Bachelor's degree required
  • Master's degree in Nursing, Health Administration, or Business Administration preferred
  • 5+ years’ experience in Case Management Leadership, preferably within a large multi-hospital system required 
  • 7+ years overall Clinical Case Management experience
  • Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation. 
  • Certification in Case Management 
  • Licensed Clinical Social Worker
