You will be supported by a multidisciplinary team, including medical professionals and support staff, dedicated to maintaining a high standard of care.

• Assist clients in navigating the intricate healthcare systems to promote integrated healthcare that addresses both their mental and physical health needs.
• Provide education on community resources and services available to clients, empowering them to participate actively in their own health management.
• Actively attend and contribute to multi-disciplinary treatment team meetings, ensuring patient care is coordinated and comprehensive.
• Document client interactions thoroughly in clinical records and other needed areas to ensure continuity of care and compliance with regulatory standards.
• Employ evidence-based screening tools to accurately assess the healthcare needs of clients and share findings with the treatment team for informed decision-making.

The Licensed Clinical Social Worker will primarily work with diverse clientele, including individuals dealing with chronic illnesses, those navigating end-of-life processes, and patients in need of mental health support. We serve a varied population, providing tailored interventions that meet the unique cultural and personal needs of each client. Your role will be pivotal in advocating for and providing holistic care to enhance their overall wellness.

• An accredited Master's degree in Social Work, Counseling, or a closely related field.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Pennsylvania.
• A minimum of three years of relevant experience working in an outpatient or similar healthcare setting.
• Exceptional verbal and written communication skills, crucial for effective interactions with clients, families, and other professionals.
• Ability to thrive within a collaborative interdisciplinary team environment.
• Demonstrated capability to manage a varied workload effectively in a fast-paced setting.

• Experience working in hospice, palliative care, or similar environments will be highly regarded.
• Familiarity with telehealth practices and remote patient care methodologies is advantageous.
• Knowledge of evidence-based practices and tools specific to social work in healthcare settings.

The salary for this position ranges from $90,000 to $100,000 per year, commensurate with experience, alongside a comprehensive benefits package.
