About the Role:
Caring Minds Health Services LLC is looking for a dedicated and compassionate licensed Adult Day Home (ADH) or Child Day Home (CDH) provider to join our growing team in Peoria, AZ. This is a rewarding opportunity to make a meaningful difference in the lives of individuals and families by providing quality care in a home-based setting. What you will do
  • ย Provide a safe, stable and welcoming home environment.ย 
  • ย Support the member with daily living activities, personal care, habilitation and supervision.ย 
  • ย Encourage independence, choice, dignity and community involvement.ย 
  • ย Follow the memberโ€™s Person-Centered Service Plan.ย 
  • ย Assist with appointments, transportation and community activities when required.ย 
  • ย Maintain required documentation and promptly report incidents or changes.ย 
  • ย Work cooperatively with Caring Minds, the member, guardian and planning team.ย 
  • ย Maintain licensing, training and life-safety requirements.ย 
Minimum qualifications
Applicants must:
  • ย Be at least 21 years old.ย 
  • ย Have a suitable home and appropriate bedroom space for a member.ย 
  • ย Have sufficient household income or resources apart from developmental-home room-and-board payments.ย 
  • ย Be available to provide the care and supervision required by the member.ย 
  • ย Not operate daycare services from the home.ย 
  • ย Be willing to complete fingerprinting, background checks, reference checks and health self-disclosures.ย 
  • ย Participate in a home study and life-safety inspection.ย 
  • ย Complete required CPR, First Aid, Article 9 and developmental-home training.ย 
  • ย Maintain a valid driverโ€™s license, vehicle registration and insurance when providing transportation.ย 
  • ย Ensure adult household members participate in required licensing and background-check procedures.ย 
Arizona DDD requires developmental-home applicants to be at least 21, possess income or resources independent of room-and-board payments, avoid outside employment that conflicts with member supervision and refrain from operating daycare in the home. Applicants and adult household members are also subject to background, fingerprinting and home-study requirements.ย
