Hospice Liaison
Promotes Halifax Health, Health services (hospice, palliative and supportive care) in the community through educational in-services. Serves as a liaison with physicians' offices, health care facilities, hospitals and community groups. Provides education regarding continuum of care services (i.e. hospice, palliative and supportive care).
JOB QUALIFICATIONS:
  • Knowledge of Hospice philosophy of care. Medicare conditions of participation; State of Florida regulations governing hospice; hospice operations and sales and marketing.
  • Healthcare experience, medical sales and marketing experience preferred.
  • Good written, verbal and public speaking skills required.
  • Possession of valid Florida driver license and clear driving record.
  • Bachelor's degree in related field or higher or equivalent years of experience.

DUTIES AND RESPONSIBILTIES:
  • Establish and maintain good working relationships with referral sources to include, but not be limited to: physician, hospital case manager, nursing home staff, assisted living facility staff and other allied healthcare professionals.
  • Educate and be an ongoing resource of skilled care and end-of-life care and services offered by Halifax Health-Hospice and to the medical and non-medical community.
  • Educate and be an ongoing resource of Halifax Health system services.
  • Develop plans of care for referral sources based upon referral practices and identify barriers to early referral. Develop and maintain good customer knowledge base.
  • Act as liaison between referral sources and clinical operations of health system. Provide feedback to ensure optimal customer satisfaction and patient outcomes.
  • Ability to document customer compliments and complaints into the Halifax EMS system.
