Key Responsibilities
  • Conduct thorough and ongoing assessments of clients to identify their unique mental health needs and develop tailored treatment plans.
  • Communicate regularly and effectively with supervisors, clinicians, family members, and guardians to discuss client progress and treatment effectiveness.
  • Maintain an organized schedule of client appointments, home visits, and observational sessions to track patient advancements and outcomes.
  • Coordinate and execute home visits when necessary for in-depth observations, chart reviews, and to gather relevant data that informs care strategies.
  • Ensure the proper implementation of program plans while actively participating in Interdisciplinary Team Meetings to align on client care strategies.
  • Document and complete monthly and quarterly progress notes to monitor client growth and inform continued therapeutic approaches.
  • Organize training sessions for staff to guarantee ongoing competence in program implementation and documentation.
  • Provide follow-up support through interviews and home visits, ensuring clients receive the assistance necessary to meet their ongoing needs.
  • Facilitate communication and collaboration with the public and the mental health community to develop additional support resources and services.
  • Assist in creating training materials and procedures to enhance the efficacy of our programs.
