The CSLP – Care Provider  assists with the day-to-day operations of the group home.  They carry out responsibilities in the following functional areas: client rights, personal cares, daily living activities, and regulatory compliance.    

Essential Functions and Desired Outcomes
  1. Performs duties relating to the support, care, welfare, supervision and training of persons with mental illness, developmental disabilities, and/or acute, chronic or long-term conditions.
  2. Serve as a role model and coach for clients regarding developing and using effective problem solving, decision-making, conflict resolutions, independent living skills and responsibilities.
  3. Learn the independent living skills and development needs of clients to assist clients in implementing goals and objectives for meal planning and preparation, housekeeping, laundry, proper use of household appliances and furniture, shopping, budgeting, etc. Provide training, demonstration, and observation both individually and in group sessions about identified areas. Make accommodations as needed to assure client and staff safety. Provide assistance if client is not able to complete a task.
  4. Provides personal cares as directed by individual needs in a client specific manner.
  5. Assists and monitors all activities of daily living as indicated in support plans and as desired by the client to include, but not limited to: personal hygiene, toileting, and independent living skills.
  6. Teaches and guides clients in achieving hopes, dreams, goals and objectives through self-direction, decision-making, and goal setting.
  7. Advocates, enforces, and monitors the protection of consumer rights to ensure the proper treatment of consumers. 
  8. Follows safety and regulatory procedures in all areas of food preparation, serving of meals, clean up and other related activities.
  9. Prepares and serves meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure client health.
  10. Monitors and documents medications of clients to ensure the client is administering their medications as prescribed.
  11. Follows all documentation and reporting policies and procedures for any medication and/or safety issues, and significant events.
  12. Coaches, monitors, and reports client’s verbal and physical behaviors to ensure the safety and health of clients and others.
  13. Assist the clients in maintaining a safe, sanitary and welcoming home by performing homecare maintenance and other domestic duties to include, but not limited to cleaning, laundry, etc.
  14. Responds promptly and competently in emergency response situations.
  15. Assists, escort, and provide physical and other types of assistance needed by client during social and recreational activities.
  16. Ensures continuous information exchange between staff, case managers, guardians, family members, funding agencies, and service providers, this is done by phone, email and meetings as required.
  17. Provide individual services to the clients that are outlined in their care plan, following the authorized hours.
  18. Possess physical aptitudes and capabilities that will enable performance of the job to include: ability to stoop, stand, pull, push, hear, see, kneel, walk, grasp, reach, talk, and lift at least 35 pounds. 
Education/Qualifications
  1. Must possess a high school diploma/equivalent and/or related work/life experience.
  2. Must be at least 18 years of age.
  3. Must possess a valid WI driver’s license, and ability to meet agency driving standards.  Must provide proof of auto insurance.
  4. Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
  5. Knowledge of and ability to intervene appropriately in crisis prevention and de-escalation when necessary.
  6. Experience working with people who have sever and persistent mental illness preferred.
