Job description

Position Summary

Collaboratively with the Team Leaders, Nurses, and Member Advocate, works to help ensure members can successfully complete routine activities of daily living while promoting dignity and independence in the process. The DCW is a critical part of the Adult Day Health Services team, working closely with our members providing person center services that are focused on bringing happiness to the lives of those we are privileged to serve.

All Pool Direct Care Workers must be available to work in all adult day facilities. We currently have facilities in Tempe & Glendale, Arizona.

This position does not have direct reports.

Essential Functions:

Care Delivery

Delivers care by displaying a positive, can-do attitude that conveys enthusiasm, compassion, and understanding to the members lives by performing the following:

  • Assists or leads group discussions, sensory stimulation, exercise, or crafts and implementation of calendar events
  • Provides members stand by or full assistance with participating in individual and group activities
  • Participates in care plan meetings for members
  • Writes progress notes and documentation on member activities
  • Takes inventory of supplies and informs team lead when supplies need to be replenished or purchased
  • Assists with helping to facilitate center outings and/or special events
    • Observes, charts and reports to Center Nurse, Lead DCW, or Center Director conditions, reactions, and responses of members as related to assigned duties
    • Assembles and disassembles equipment and accessories

Health, Safety & Security

Assists with the health maintenance, care, and activities of daily living of members according to the care plan by performing the following:

  • Monitors the safety and security of members at all times
  • Provides assistance to members in need of using the restroom and, as needed, with routine toileting care and when necessary changing articles of clothing, laundering as necessary
  • Based on the individual needs of each member, provides bowel and bladder care, restorative feeding and therapy, ambulation, range of motion, personal living skills, transfers, and showering as needed and in accordance with training
  • Obtain member’s blood pressure and weight
  • Helps with the serving of food according to licensure and CACFP guidelines
  • Assists members with getting in and out of vans or private cars; transferring members from wheelchair to chair
  • Works in a team-oriented manner to ensure the Center is disinfected in accordance to the Center’s Policies and Procedures to include ensuring all activity areas, kitchen, bathrooms, and shower areas are kept neat and clean, including mopping floors and wiping down surfaces after the space has been utilized

  • Ensures compliance with all AllThrive 365 policies, procedures, and practices regarding the Adult Day Health Services program, and other contract and licensure requirements; and applies knowledge to the work in accordance with quality standards
  • Attends and actively participates in initial and on-going training and development programs and opportunities, weekly unit meetings all staff meetings, and demonstrates learned concepts on the job
  • Participates in assigned work teams, as requested and appropriate
    • Engages in continuous improvement efforts, as assigned, and strives to continuously improve the quality of care and self
    • Communicates daily and as necessary with colleagues, nurses, supervisor, and leadership

Requirements

Education & Experience, Knowledge/Understanding

  • High School Diploma or General Education Diploma
  • No experience necessary
  • Article 9 Certification needs to be completed prior to working independently with members
  • Valid CPR/First Aid Certification or ability to obtain within 30 days of position entry
  • Valid Food Handlers Card or ability to obtain within 30 days of position entry
  • Must be at least 18 years of age
  • Clear fingerprint background screening through the Department of Public Safety and comprehensive background clearance prior to new hire orientation

  • Strong verbal, written, reading, and listening communication skills to include communicating clearly, effectively, tactfully, and patiently to a diverse population.
  • Strong interpersonal skills and demeanor with the ability to build rapport and garner trust with others.
  • Proficient in the use of a PC; in a Windows environment; in the use of the Internet; and in the use of MS Office Applications such as Outlook, Team, and Word.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.

  • Works in a climate-controlled environment, infrequently outdoors
  • Able to lift 50 pounds
  • Requires bending, stooping, lifting and standing for long periods of time
  • Able to work under conditions of frequent interruptions
  • Able to work with challenging behaviors expressed by members, i.e. verbal and physical aggressions, wandering, etc.
  • May be exposure to potentially infectious fluids such as blood or body fluids
  • Exposure to latex, plastic, and/or materials which are used for personal protective equipment

Preferences

  • Certified Nursing Assistant (CNA)
  • One year paid or volunteer experience working with older adults, adults with living with dementia or a cognitive disability, or adults living with a mental health diagnosis
  • Experience assisting with planned group activities or recreation
  • Bilingual (English/Spanish)
