Job Description TTM Healthcare are currently recruiting a Clinical Nurse Manager 2 - Discharge Coordinator on behalf of a South Dublin based private Hospital that is renowned for career progression opportunities and excellent benefits package. The Role: The post holder will use clinical expertise, leadership and management skills to ensure service provision and timely discharge from hospital. Discharge co-ordination is fundamental to creating essential capacity across the hospital in order to manage patient flow through the hospital effectively.

The primary role of the Discharge Co-ordinator will be one of clinical and professional leadership to ensure the patients continuum of care to discharge is managed from time of admission. The discharge coordinator will also manage and develop the nursing team within the patient lounge and have direct oversight of patient lounge activity. The Discharge Co-ordinator will develop strong relationships with all members of the multidisciplinary team to coordinate safe and timely discharge from hospital.

This is a full-time permanent role working 37.5 hours per week.
