Job description

Custom Care Pharmacy is seeking a motivated Graduate Pharmacist Intern to join our closed-door long-term care pharmacy in Milton, Pennsylvania. This is a delivery-only LTC pharmacy, not a traditional retail setting, so you can focus on workflow, accuracy, patient care, and meaningful clinical work without front-end customer traffic.
We are a fast-paced, team-oriented pharmacy that values hard work, growth, and innovation. This is an excellent opportunity for a new graduate who wants strong hands-on experience in long-term care pharmacy and wants to grow in an environment with real clinical involvement and future opportunities for expanded pharmacy services.
Position Overview
As a Graduate Pharmacist Intern, you will work closely with pharmacists, technicians, and leadership to support daily pharmacy operations in a high-volume LTC setting. You will gain experience in medication order review, packaging workflows, facility communication, transitions of care, and quality-focused pharmacy operations.
Key Responsibilities
  • Review medication orders for appropriateness, accuracy, and safety
  • Assist with prescription processing, order entry, and workflow support
  • Support LTC operations including cycle fills, new admits, medication changes, and STAT orders
  • Help ensure accurate dispensing, packaging, and delivery preparation
  • Communicate with nurses, facilities, prescribers, caregivers, and pharmacy team members
