Job description

The High Risk Breast Program at Sentara Brock Cancer Center is a specialized outpatient clinic dedicated to risk assessment, education, and proactive care for individuals at elevated risk for breast cancer. The Advanced Practice Provider (APP) is responsible for the comprehensive assessment, management, and longitudinal follow-up of individuals at elevated risk for developing breast cancer. Working collaboratively with a multidisciplinary team, the APP provides evidence-based risk assessment, screening, prevention, surveillance, patient education, and care coordination services designed to promote early detection and reduce breast cancer risk.

The APP will provide care for patients across the continuum of risk management, including:

  • Individuals identified as high risk through validated risk assessment models (e.g., Tyrer-Cuzick, Gail).
  • Patients with hereditary cancer syndromes or known pathogenic genetic mutations (e.g., BRCA1/2, PALB2, CHEK2, ATM).
  • Individuals with significant personal or family histories of breast, ovarian, pancreatic, prostate, or other hereditary cancers.
  • Patients seeking education and counseling regarding breast cancer prevention, screening, lifestyle modification, and risk-reduction strategies.

The ideal APP possesses expertise in breast health, cancer prevention, genetics, women's health, oncology, or survivorship care and demonstrates strong skills in patient counseling, risk assessment, multidisciplinary collaboration, and population health management within a prevention-focused care model

Enrollment in a high risk certification course, such as City of Hope Intensive Course in Genomic Cancer Risk Assessment, is required
