Full-Time Social Worker – Subacute Unit

Hopkins Center for Rehabilitation & Healthcare is seeking a compassionate and motivated Full-Time Social Worker to join our Subacute Care Team. We are a 5-Star facility dedicated to providing exceptional care while creating a supportive, team-oriented environment for both our residents and staff.

Responsibilities

·       Complete psychosocial assessments and social service documentation in accordance with state and federal regulations.

·       Participate in interdisciplinary care planning and resident care plan meetings.

·       Provide counseling, support, and education to residents and families.

·       Coordinate discharge planning and community services.

·       Assist residents and families with adjustment, coping, and transition needs.

·       Maintain accurate documentation within the Electronic Medical Record (EMR).

Qualifications

·       Bachelor's Degree in Social Work or related human services field required; Master's Degree preferred.

·       Minimum one year of experience in healthcare, rehabilitation, or long-term care preferred.

·       Experience with discharge planning and interdisciplinary team collaboration.

·       Strong communication, organizational, and interpersonal skills.

·       Proficiency with Electronic Medical Records (EMR).
