Are you passionate about supporting others and making meaningful connections? Glacier Hills is seeking a dedicated Full-Time Social Worker to join our Social Services Department and play a vital role in enhancing the lives of our residents and their families.

About the Role

As a Social Worker at Glacier Hills, you will be an advocate, resource, and trusted partner in care. Your responsibilities will include:

  • Participating in discharge planning, resident assessments, and the development and implementation of individualized social care plans
  • Conducting cognitive and mood assessments and coordinating appropriate follow-up care and treatment
  • Completing timely and accurate documentation, including medical forms, reports, evaluations, and charting
  • Referring residents and families to community resources and appropriate social service agencies
  • Educating residents and families on Medicare, Medicaid, and other financial assistance programs
About You

You are compassionate, organized, and committed to resident-centered care. Qualified candidates will have:

  • Active Social Worker licensure in the state of Michigan
  • A Bachelor's or Master's degree in Social Work or a related human services field (such as Sociology, Psychology, Special Education, or Rehabilitation Counseling)
  • One year of supervised social work experience in a healthcare setting (preferred)
  • Experience working in long-term care or with the geriatric population (preferred)
