Position: Skilled Registered Nurse (RN) in Skilled Nursing Unit

  • Sign on bonus - $1,000 for Full time and $500 for Part time.

Shift Schedule:

  • Full-Time NOC Nurse: Shift 10 pm to 6 am
  • Full-Time AM Nurse: Shift 6 am to 2 pm

Responsibilities:

  • Conduct health assessments and create personalized care plans for each resident.
  • Administer medications and treatments according to physician orders and established protocols.
  • Monitor residents' vital signs and promptly address any changes in health status.
  • Collaborate with interdisciplinary teams to provide holistic and person-centered care.
  • Assist in the development and implementation of wellness programs and activities.
  • Educate residents and their families on health-related topics and promote preventive care.
  • Respond to medical emergencies and provide immediate and appropriate interventions.
  • Maintain accurate and up-to-date resident medical records.
  • Foster a supportive and compassionate environment for residents and their families.
  • Participate in ongoing training and professional development opportunities.

Qualifications:

  • Skilled RN with a proven track record of managing 1-29 skilled and long-term residents.
  • Full time RN/LPN on AM shift
  • Can work 8 days a pay period and Willing to work every other weekend
  • With skilled care experience that is willing to work in either Assisted Living or skilled unit
  • Has taken care of 20+ skilled or 35+ assisted living residents
  • Point Click Care experience
  • Knowledge of basic wound care
  • Experience in Fast-paced environment, great building, and great team.
  • Proficient in medication pass, IV administration, g-tube, and complex clinical care management.
  • Experienced in wound care, families, doctors, other vendors, and changes in condition.
  • Night RN: Independent RN with night shift experience. Ability to hold staff CNA accountable and assist the Assisted Living nurse if needed. Capable of managing fall incidents, changes in condition, accessing emergency support, notifying MD, and keeping management informed.
  • Willing to float if needed.
  • You love working with people and handle everyday challenges with a smile!
  • Graduate or Future Graduate from an accredited nursing program
  • Hold a Current State License as a Registered Nurse (RN)
  • Able to communicate well in both verbal and written forms
  • Have a minimum of 1 year experience as a Registered Nurse (RN) in a long term care setting/Skilled Nursing Unit
  • A natural problem solver
